Guys like Lepke and Lansky were only high level non-Italian gangsters who cooperated with US Cosa Nostra on daily basis and they also attended meetings on which faces such as Luciano and Costello were present. The US CN was always different from the European model simply because the whole country was made of various nationalities which constantly cooperated with each other, even today, no matter if legit or not. So if someone wants to call a dozen of meetings between the previously mentioned criminalsfrom different nationalities a "second commission", let it be since there was only one real commission and it was all Italian. Me personally, I would rather name the close connection between the Italian and non-Italian gangsters simply as a "clique", instead of a "second commission". I would also rather believe in a one time second and lower level Italian commission on the Midwest, possibly controlled or represented by Chicago
