I agree with TommyG. John Gambino has incredible clout on both sides of the Atlantic for decades, we're talking almost 40 years here. If this was the 70's they probably would've been more open about his position of authority, but in this day and age a boss can only operate successfully from the shadows and make it seem other people are boss and leave LE, the media (and nosy people like us ) in a state of constant confusion.
