Wasnt Massino stopped because wanted to induct 10 men in the same ceremony ? I think that isnt more an approval but a simple notification so the other families top hierarchies know who's who. I know that there are limits ie any family can replace only the old members and can't induct too men in a same ceremony for don't made a family too strong.
