I think the current hierarchy as of 2015 is one of two possibilities:
(I think John DiFronzo, Pete DiFronzo and Joseph Andriacchi are all retired). Possibly Marco D'Amico as well.

Option 1:

Boss- Sal DeLaurentis
Underboss-Sal Cautadella
Consligiere- John Matassa
Elmwood Park Capo- Rudy Fratto
Elmwood Park Capo- Tony Dote
Grand Avenue Capo- Albert Vena
Cicero Capo- James Inendino
Cicero Capo- Lou Marino
26th Street Capo- Frank Caruso

Option 2:

Boss- Sal DeLaurentis
Underboss- John Matassa
Consligiere- Lou Marino
Elmwood Park Capo- Rudy Fratto
Elmwood Park Capo- Marco D'Amico
Grand Avenue Capo- Albert Vena
Cicero Capo- James Inendino
Cicero Capo- Sal Cautadella
26th Street Capo- Frank Caruso

I think that pre 2007, there were actually two separate Outfit hierarchies. One was Elmwood Park/Grand Avenue and the other was Cicero/26th Street. Once Marcello went away in 2005-2007 and Sarno took over the Cicero crew, the Outfit was reunited with one hierarchy with DiFronzo as boss, Andriacchi as Underboss and D'Amico as Consigliere. I think from 2001-2007, I think the hierarchy looked like this.

Northside Group:

Boss- John DiFronzo
Underboss- Joe Andriacchi
Consiglieri- Marco D'Amico
Elmwood Park Capo- Peter DiFronzo
Grand Avenue Capo- Jimmy Cozzo/Albert Vena

Southside Group:

Boss- James Marcello
Underboss- Michael Sarno
Consiglieri- Tony Zizzo
Cicero Capo- James Inendino/Sal Cataudella
Melrose Park Capo (part of Cicero)- John Matassa
26th Street Capo- Frank Caruso
