it could still show michael before he died
I would still find that very boring. The Godfather trilogy showed Michael in his prime, making his bones and doing anything exciting during his life basically. What would be left so show?
But the film could be primarily based around the family helping Michael through an illness, and crime-related problems surrounding the family during michael's illness. In the end, it would catch up to the last scene of GFIII, filling a major plot-hole. It would, in a way, mirror the original GF, in which Vito is shot and must be tended to.

"Somebody told me when the bomb hits, everybody in a two mile radius will be instantly sublimated, but if you lay face down on the ground for some time, avoiding the residual ripples of heat, you might survive, permanently fucked up and twisted like you're always underwater refracted. But if you do go gas, there's nothing you can do if the air that was once you is mingled and mashed with the kicked up molecules of the enemy's former body. Big-kid-tested, motherf--ker approved."
