It is for authenticity and nothing else that it should be in Sicily and New York.
True, inside scenes could probably shot anywhere (for that matter maybe some outside scenes as well).
However, like I said before, if your making a movie about the Sicilian mafia....go to Sicily.

We here are all GF fans and do pay attention to detail such as where GFIV may be filmed. What can I say, we are all addicted.
