Rastelli never really had an underboss per se. He was in prison for a long time, and while he was away, Carmine Galante was maneuvering to be the Bonanno boss--actually declared himself the boss. Massino was a caporegime, along with a cast of others: Dominic (Big Trin) Trinchero, Alfonse (Sonny Red) Indelicato, Dominick (Sonny Black)Napolitano and, after Galante was whacked, Cesare Bonventre. Sal Catalano actually became acting boss for a very short while, but he was disqualified because he was a Sicilian--in a bizarre twist, only an American born Mafioso could be the boss. Massino and Napolitano whacked Big Trin and Sonny Red. Then Sonny Black was whacked after the Donnie Brasco affair, then Bonventre was whacked on Massino's orders. That left him the last man standing under Rastelli when Rastelli got out of prison; and therefore, his underboss.

There is a lot of controversy about who pulled triggers in that massacre--but all are agreed that McGurn wasn't one of them. He planned the massacre mainly in revenge for having nearly been killed in a trap laid by Moran's two chief thugs--the Gusenberg brothers. He sold Capone on the idea by convincing him that Moran could be erased in the same hit. To protect himself, McGurn holed up in a hotel with his mistress Louise Rolfe (known as "the blonde alibi"), and kept calling for room service at the time of the shooting to reinforce the alibi.

According to Capone's most recent biographer, Lawrence Bergreen, the shooters were Fred (Killer) Burke, Albert Anselmi, John Scalise and Pasquale (Patsy) Lolordo.
