So I love the film , and in fact they did leave the ending open with the possibility of a sequel. We assume Carlito dies at the end, but its not confirmed. They could have easily saved his life and we could have been given a sequel.

If in fact a sequel would have been put together, what would you have liked to see regarding the storyline? In my opinion the sequel would have started by showing Carlito retired with a daughter living somewhere tropical such as South Florida (Miami). This also would have made a connection for the fans of the film since so many were big fans of Scarface as well. You could have even tied in some scenes with similar locations that were used in film Scarface such as the hotel pool where Manny and Tony are having tropical drinks.
