Did any sort of organized pyramid structure exist before Maranzano and then Luciano formed the Mafia in 1931? IIRC it is then that these organizations begin calling themselves 'families' rather than 'gangs'.

But I'm asking about the ranks (Capo, Underboss, Consigliere, etc.) Were they in use in the Italian mob before 1931? If not, what ranks were there? I imagine that a boss like Masseria or Torrio would have lieutenants, so how were these positions called?

The same as after 1931. 1931 simply marked the organization of the Commission.
