Mad Dog DeStephano.

This mob boss was replaced as head of one of the five NYC families because he was considered to be too close to the slain Salvatore Maranzano. He was eventually killed twenty years later.

If by "outfit" you mean the Chicago mob, it's your namesake.

Accardo: Wrong, Castellano was indicted but didn't go to trial because he was killed.
My question stands: Which of the five Dons wasn't indicted in Giuliani's RICO blitz, and why wasn't he indicted?

Ok, since it's more than 24 hours:
Vincent (Chin) Gigante was the only Don who wasn't indicted in 1985 in the Great Giuliani RICO Blitz. Two reasons: 1) Gigante had secretly taken over the Genovese Family in '81 after Fat Tony Salerno had a stroke, but the Feds still thought Salerno was in charge; 2) Gigante had started his "mentally incompetent" act.

New question: What was Joe Columbo's "legitimate" job?

He meant the Mayfield Road gang from Cleveland: Big Al Polizzi, Moe Dalitz, Sam Tucker and Morris Kleinman.
Q. Roth refers to "Eddie Levine of Newport." Who was the real-life equivalent?

Didn't think there was a real-life equivalent, that he was ficticious but could it have been Carlos Marcello?

No, not Marcello (although Marcello did business with Meyer Lansky, Roth's real-life equivalent). Any other guesses or answers about "Eddie Levine of Newport"?
