The clan is headed by Carmine Montescuro, nicknamed o 'munuzz'.

The interim order reconstructs numerous relationships related to the Montescuro clan and intended to ensure the organization of illicit proceeds, in particular extortion to merchants and entrepreneurs operating in the Port area. From the elements reported in the precautionary provision it appears that extortion was also committed against the entrepreneurs involved in the renovation and maintenance of the roadway which starts from Via Marina and also involves the port area up to the eastern area. The proceeds of the extortions to the construction sites committed by the Montescuro clan have been divided among the various criminal organizations, recipients of a quota determined on the basis of the influence on the territory. In this way, Carmine Montescuro would have succeeded in maintaining the balance between the various associations, avoiding the emergence of conflicts, and at the same time guaranteeing the regular execution of extortion activities and the participation of all in illicit profits, so that some collaborators of justice, by virtue of their neutral position, they indicated that the Montescuro clan operated in S. Erasmo, as, indeed, a 'small Switzerland'.
