To answer the original question besides the Five Families you have Chicago,New England,Philly-South Jersey,DeCavalcante's, and like Ivy said maybe Detroit, outside of there it's mostly collapsed but with considerable numbers in Kansas City and Buffalo remaining with arguably more signs of like from KC, outside of that even I'd guess there's a few guys in places like Tampa,LA,Cleveland and Sonny Ciancuttti's one man show in Pittsburgh, I think thats about it though
