Anthony Casso was a real psycho and he had brains. That's a dangerous combo. Galante was feared but not feared enough to avoid getting clipped. Guys like Anastasia and Genovese were no picnic either. Toto Riina would slaughter babies... Most feared, I'd say The Chin for the simple fact that he tried to kill Costello and nobody dared touch him when he was the boss of the Genovese.

In terms of smarts and overall power maybe Carlo Gambino. I mean on the surface he didn't look threatening but he held real power. In terms of looking at the guy and shitting yourself, I'd say Casso, Gravano, or Gigante

Obviously as someone already said Galante and neil were scary because they were killers but also powerful. Then if you look at regular soldiers you have roy demeo, tony mirra,tommy pitera, tony plate, and tommy desimone who were considered psychopaths even within the mob

Maybe not scary looking but he was close to 7 feet tall a known killer and was Carmine's bodyguard to me that's reason enough to include him. Other people have said Gambino,Ricca,Accardo but you thought you'd point out Cesare isn't scary looking

Thats a tough one to pick. I am biased towards Pitera so I will go with him. Anyone who enjoys standing naked in a bathtub cutting up bodies and getting blood all over him, and feels comfortable doing it around other people, is all I need to make my decision. I wouldn't doubt the fact that he also drank the blood of his victims. Sort of like how hunters drink the blood of a dead deer after they make their first kill.
