It depends imo there's been times when the Underboss has merely been a lieutenant to the boss for instance Sammy Gravano. There's also been times the Underboss has directly controlled crews like Delacrocce controlling crews in Manhattan. There was also the rare case of Casso who was more like a street boss.

The Consigliere position has also greatly changed it seems. It seems that in the 30s,40s the Consigliere was more like an advisor but now it's just an extra lieutenant .
