Most likely he's not being extradited to Italy because mafia association is not a crime in the US or Canada.
I've always been curious about that Mafia Association law in Italy. Exactly what does someone have to do to be guilty of "Mafia Association"? Talk to a known Mafioso? Seen in public with one? In business with someone? Seems kind of vague to me.
Pretty much, yes. In Italy, they will come and arrest anyone they even think has been doing business with a mafioso. If you are a store owner and paying to the mob, or even just approached, and don't report it, you can also be arrested now since this forza italia has come into effect. If you are a family member and they even suspect you have had contact with a fugitive mafioso in your family, they will arrest you. You'll be held til you can prove your innocence. Guilty until proven innocent. That's the way it is.
