I personally think that it isn't a dream sequence and that Tony is in purgatory. My family since the death of my grandmother has always been wondering about such life after death and Tony I feel is in purgatory. He can't get out of where he is. He has lost his identity. The fires is costa mesa and that commercial all lead me to believe this.
