There is nothing that says, in mobdom of course, that one must hold each title/postiton in the chain of command to become a Don.

Like any other structured organization, it is preffered that a person who is going to be given a high postion work his way up the ladder, holding titles at different levels in the chain of command. But we see it everyday in the business world. We see people who've worked for 20 years in a company, working their way up the ladder and holding various high ranking positions and titles each time they advance up the ladder and then one day some young, inexperienced college kid comes walking in and says
" Hello, I am your new boss!" And it turns out that he may be the nephew of the President of the company.

If John Gotti took his son under his wing, and taught him the business, there is no rule that says that John Jr. had to hold a postion at each level in the family structure before being named the Boss of the family.

Come to think of it when Gotti Sr. decided to have Big Paul wacked, he ( Gotti ) was not the next in line to be the new Don. He had him wacked and then just took the position because of an alliance that he formed with other bosses and capos before taking Big Paul out of the picture.
