In the book, it says, "I am Stefan Andolini, your father and I grew uo tegether in Corleone. I saw you in America, when you were a child." So I'd say he was Vito's cousin. But then he also calls Michael, "Cousin Michael", but I suppose that can still work, as I call my dad's cousins my cousins too. Hope that helps, Liz.
