The undisputed clan chief of the "family" of San Giorgio a Cremano is Filippo Abate, nicknamed "Tore 'or horseman," the star of a sensational escape from prison in Rotterdam at the end of February '93. For two years he was able to hide in the Netherlands, then moved to Brazil, where the work of intelligence worked perfectly. He was captured on a beach in Salvador de Bahia by the local Federal Police, which collaborated in a decisive manner the police of Naples. The birth of the clan and his rise to the top of the hill dates back to 1984, when the then young prejudiced Giovanni Alfano was wounded in an ambush by the Camorra. Alfano escaped with a fortnight of hospitalization at the Loreto Crispi.

The organization of Alfano grew dramatically thanks to alliances with the Picuozzis- Marianos of the Spanish Quarter, with Frizzieros of the Torretta, with Sarnos and the mala of Posillipo, the Paesanos and Calones.
The clan got billion (lire) for extortion in the hospital to businesses and contractors to work in hospitals, cleaning and canteen. According to investigators, the clan did not exclude anyone from requests for bribes, so they would pay protection money also the contractors of Antonello Venditti and Claudio Baglioni for the concerts Collana and Sao Paulo stadiums
Another source of income for the clan was drug trafficking. Alfano finished the boss on trial for complicity organization of drug dealers who ran the sale of drugs to the VIP feasts of Naples .
