In the New York Cosa Nostra you have this structure.
Capo di tutti capi
Boss(in each of the 5 families)
Consigliere
Underboss
Caporegime
Capodecime(does this position exist?or you can say just caporegime?)
soldier
associate

What is the structure of Camorra Napolitana and Ndraghenta?

It's not Ndraghenta, it's Ndrangheta.

You just pronounce it like it is, but you say the 'e' a bit longer (like Fanucci's 'eeeeeeh', but maybe a little shorter than that ). And you gotta have a rolling 'r', an Italian 'r'.
And of course you can't say the 'a' like in English. So no 'a' like in baker, but an 'a' like in fAnucci.
The emphasis in Ndrangheta is on the first 'a'.

Hope this will help you...

The word derives from greek "andragathía": heroism, power
