Can someone provide some detail on the specifics of how gambling works in the modern day LCN?

Where are card/dice games held? Are the same spots used frequently or are floating cards more popular? What kind of money is thrown around? Do gamblers have to pay to get in or is the money earned by taking a percentage of the pot? What kind of people attend? How do bookmaking operations work? Are they all off-shore sports books or do they also still use more traditional methods? What kind of places do bookies use as their headquarters? How does loansharking tie in?

I think you get the picture. It'd be awesome if someone could give me some insight on some of this on operations varying in size. Any relative info is appreciated.
